You should shortly receive an email with a PDF attachment based on your responses. Please carefully follow the instructions below to complete the processing of this agreement:
- Obtain signatures from the UBC researcher, affiliated hospital, and the data provider and/or receiver. You are responsible for obtaining these signatures to this agreement.
- Once these signatures are obtained, send the partially signed agreement to sponsoredresearch@innovation.ubc.ca for UBC’s signature. Note that faculty and staff are not authorized to sign on behalf of UBC or the affiliated hospital.
ANY MODIFICATIONS MADE TO THE DATA TRANSFER AGREEMENT WILL RENDER THE AGREEMENT INVALID.
